We share space with GrassRoots Functional Medicine in West Lebanon, NH. You can check in at the reception desk and they will point you in the right direction.

We are an out-of-network practice and accept only cash-based payments. What we can offer is an itemized receipt called a Superbill, which you can personally submit to your insurance company for possible reimbursement. Restore the Floor Physical Therapy is not credentialed by Medicare, thus only services that are not covered by Medicare can be provided. Superbills cannot be submitted for Medicare coverage.

Venmo, Check or Cash are all acceptable payment methods

Great Question! I'm a Mom - and although I love what I do, my family comes first. There may be a season where I increase my hours, but while my kids are still young I feel that my place is primarily at home. 

Please know that I value both your time and my time. Once you have booked an appointment, that time is set aside to focus on you and your physical therapy goals. 

I understand that there may be appointments that need to be rescheduled or canceled. Because I am a sole provider and have limited availability, I ask for at least 72 hrs notice (the Friday before your appt) so that I can offer the appointment time to another patient who needs to be seen.

For any cancellations within 72 hrs, 50% of the appointment cost will be charged. Patients who no-show will be charged the full cost of the visit.
